Why Choose Dr. Burgess for Your Preservé™ Breast Augmentation?

Breast augmentation requires careful planning because implant size, profile, placement, and surgical technique all influence your final contour. Dr. Elisa Burgess has more than 20 years of practice with breast augmentation experience and holds board certification from both the American Board of Plastic Surgery and the American Board of Surgery. She specializes in cosmetic and reconstructive breast surgery so this technique is a natural cumulation of all her training and experience.

Dr. Burgess evaluates your breast tissue, chest wall, proportions, and aesthetic preferences before recommending an implant and surgical plan. This individualized approach focuses on creating a result that fits your body rather than simply choosing an implant by cup size.

Burgess Plastic Surgery and MedSpa also offers Crisalix 3D imaging so you can visualize different implant styles and volumes before surgery. The practice also offers Mindray high-resolution ultrasound for in-office silicone implant monitoring, giving patients another tool for long-term breast implant care.

What Is Preservé™ Breast Augmentation?

Preservé™ by Motiva® takes a tissue-preserving approach to breast augmentation. Instead of relying on more extensive tissue dissection to create the implant pocket, the technique uses specialized instruments and controlled tissue expansion to make space for the implant above the pectoral muscle. This is considered the posterior lamellar space. The lamella is soft tissue that naturally wraps around the breast gland tissue. The lamellar wrap goes in front (anteriorly) and in back of the gland (posteriorly). The implant is placed in this space in front of the pectoralis muscle with fascia and behind the breast gland. The space is surrounded by supportive strands of natural tissue called the circumammary ligaments. This creates a little pocket or nest to hold the implant without disruption of the tissue. This nesting technique allows the implant to move naturally with the breast.

The Motiva SmoothSilk® implants have a special surface that is considered more biocompatible. So your body may not form as much of a capsule or response to the implant. The body may see it as a more natural surface. This has been extensively studied in prestigious, scientific journals, such as Nature. The goal is for your body to acknowledge the implant but with less response and fewer complications.

For appropriate patients, this approach may provide a smaller incision, less tissue disruption, and a smoother early recovery than conventional breast augmentation. Dr. Burgess offers two primary Motiva® silicone implant styles, allowing her to personalize breast shape, projection, and movement: Motiva SmoothSilk® Round and Motiva SmoothSilk Ergonomix®.

Motiva SmoothSilk® Round

Motiva SmoothSilk® Round implants maintain a round shape and provide more visible upper-breast fullness and projection. They may appeal to you if you prefer a fuller, perkier contour with a soft feel.

Motiva SmoothSilk Ergonomix®

Motiva SmoothSilk Ergonomix® implants respond to body position, appearing rounder while you lie down and creating a more naturally sloped shape while you stand. They may suit patients seeking softer, subtle enhancement with movement that resembles natural breast tissue.

Dr. Burgess will determine which implant option, size, and profile best fit your anatomy and Preservé™ surgical plan.

How Preservé™ Breast Augmentation Is Performed

Preservé™ uses a specialized sequence to create the implant pocket with controlled tissue expansion. Through a small incision, a channel-separating instrument establishes a pathway for the pocket. A specialized inflatable balloon then gently expands and elongates the tissue to create space while limiting disruption to the native breast structures.

Dr. Burgess then places the implant into the prepared pocket using a specialized insertion sleeve. The technique allows the native breast tissue to settle around the implant rather than requiring the broader tissue dissection associated with some traditional breast augmentation techniques. Dr. Burgess confirms implant position, breast contour, and symmetry before closing the incision.

Sometimes a hybrid breast surgery will need to be done for very thin patients and limited breast tissue. This can be accomplished with fat grafting or possibly donor fat called alloClae™.  This technique of adding a little fat can blend the junction of the implant superiorly to make it less visible in very thin patients. Thickness of the tissue is measured at the clinic appointment to see if this is advised.

Preparing for Your Preservé™ Breast Augmentation

Before surgery, Dr. Burgess reviews your medical history, anatomy, implant size, and profile. Follow all preoperative instructions regarding medications, smoking, transportation, and home preparation.

What to Expect During Preservé™ Breast Augmentation

Your surgical experience begins with a final review of your treatment plan, implant selection, surgical markings, and anesthesia plan.

Burgess Plastic Surgery and MedSpa generally performs breast augmentation as an outpatient procedure. After surgery, the team monitors you in the recovery area before clearing you to return home with a responsible adult. The team will give you instructions for medications, incision care, your support bra, activity restrictions, and follow-up appointments.

Preservé™ Breast Augmentation Recovery & Aftercare

Preservé™ minimizes tissue disruption, which may support a smoother early recovery for appropriate patients. You should still expect some swelling, tightness, soreness, bruising, and temporary changes in sensation while your tissues heal.

During the first 24–48 hours, focus on rest, hydration, and short walks. Many breast augmentation patients feel ready to return to desk work within a few days, although your timeline may differ. Avoid heavy lifting and strenuous exercise until Dr. Burgess clears you, and wear your surgical bra as directed.

Avoid using warm or cold compresses during recovery as temporary numbness can interfere with your ability to judge temperature safely.

What Results Can I Expect From Preservé™ Breast Augmentation?

Your breasts will initially appear fuller, while swelling and tightness can affect their early shape. As swelling decreases, the implants settle, and the contour softens and takes on a more natural appearance. Your anatomy, implant selection, and healing response will influence your result.
